Аn assessment of the main causes of the epidemic process of chronic hepatitis C in the territory of the Republic of Sakha (Yakutia) at the stage of implementation of the national program for the elimination of viral hepatitis C was conducted. The role of pathogenic variants of the MY015A gene in the etiology of hearing loss has not been sufficiently studied in the world, since the larger size of the gene (66 exons, 71 kb) suggests the search for pathogenic variants using NGS technologies, which are not yet sufficiently used in routine practice. In this regard, it is relevant to study the role of pathogenic variants of the MY015A gene in the etiology of non-syndromic forms of hearing impairments The purpose of this work is to describe the rare pathogenic variant c.7636C>T p.(Gin2546*) in the MY015A gene, found in a homozygous state in two siblings with prelingual profound sensorineural hearing loss from a Buryat family. Previously, this variant was found only in one patient in a compound-heterozygous state with another nonsense variant in the MY015A gene in Brazil and described as pathogenic. Detection of variant c.7636C>T p.(Gln2546*) in a homozygous state in Buryat siblings may indicate either a rare case of endogamous marriage or a wider distribution of this variant in the Lake Baikal region.

Biologically active substances contained in lingonberry leaves (Vaccinium vitis-idaeae L.) have antioxidant properties. In this study, the optimal extraction method was selected to obtain extracts with a high content of biologically active components with antioxidant activity from the leaves of Vaccinium vitis-idaeae, growing in Yakutia. The data obtained allow us to conclude that biologically active substances isolated by alcoholic extraction from the leaves of Vaccinium vitis-idaeae can be used in medicine to find approaches to regulating pro-oxidant processes in the human body under various pathological conditions.

Seasonal variations in the levels of thyroid-stimulating hormone (TSH), free triiodothyronine (fT3) and free thyroxine (fT4) in young men in Yakutia were studied, where strong changes in climatic parameters in the winter-spring period (from -41.8°C to -0.2°C) were noted. Seasonal variations between winter and spring were found for fT3, where in winter its levels were lower - 6.48±0.31 pmol/L than in spring - 6.88±0.1 pmol/L (p=0.005). There were no statistically significant seasonal variations for TSH (U=97; p=0.914) and sv.T4 (U=47; p=0.112). Winter-spring seasonal variations of fT3 detected in this study there are signs of polar T3 syndrome in young men in Yakutia. To search for the causes of the detected seasonal variation, a correlation analysis of the levels of TSH, fT3 and fT4 was carried out depending on daylength and atmospheric air temperature. As a result, a correlation between fT3 and fT4 with the daylight (fT3: R=0.339, p=0.03; fT4: R=-0.346, p=0.01) and with air temperature (fT3: R=0.295, p=0.05; fT4: R=-0.296, p=0.04). No correlations were found with TSH levels (daylight: R=-0.06, p=0.69; air temperature: R=-0.09, p=0.559). Thus, the residents of Yakutia have signs of polar T3 syndrome, which can be associated with both a short light day and low atmospheric temperatures in winter. The results obtained may indicate an increase in the absorption of T3 at the tissue level when exposed to cold.

Pearson syndrome (PS) is a rare multisystem disease with predominant involvement of the hematopoietic organs, pancreas and liver, developing due to a defect in mitochondrial DNA. Most often, the first clinical manifestations of Pearson syndrome in the form of anemia of varying severity appear in the first year of life. The disease was first described in 1979 by Howard Pearson, who included in this syndrome sideroblastic anemia, vacuolation of hematopoietic progenitor cells in the bone marrow, exocrine pancreatic dysfunction, and early onset of the disease, usually before the age of 1 year. According to the literature, the incidence of Pearson syndrome is 1:5000. This article presents a clinical case of a boy diagnosed with Pearson syndrome at the age of 6 months. The child had pale skin from birth and a general blood test showed severe anemia. In the myelogram: Moderate increase in proliferation of the erythroid germ with impaired maturation, diserythro and dysmegakaryocytopoiesis, moderate monocytosis, ring-shaped sideroblasts 45 %. In a molecular genetic study: on DNA material isolated from the patient’s blood cells and urinary sediment using the polymerase chain reaction of very long fragments, the patient was analyzed for the presence of mitochondrial DNA deletions in the region where most of the major changes were described (m.6380-m. 16567). DNA isolated from the patient’s blood cells and urine sediment revealed a deletion of about 3000 bp. in a homoplasmic state. The boy also has neurological disorders. Currently, the child is admitted monthly to the oncology department of the pediatric center for replacement therapy with blood components. He has been observed by hematologists together with neurologists. He also receives chelation therapy and methylprednisolone therapy on an ongoing basis. For anticonvulsant purposes: vigabatrin. Symptomatic therapy, according to the recommendations of the federal center: Courses of Riboflavin, Tocopherol (vitamin E), Coenzyme Q, Succinic acid, L-carnitine, Thiamine.

The article presents the results of a retrospective analysis of hemotransfusion complications according to the acts of official investigations of the commissions of the Ministry of Health that arose during the transfusion of blood components in hospitals of the Republic of Sakha (Yakutia) in the period 1991-2023. The structure, causes and outcomes of hemotransfusion complications are presented. The most common mistakes leading to the development of posttransfusion complications, up to a fatal outcome, were identified. It was found that in the vast majority of the cases, violations during hemotransfusion were observed in district hospitals (70.3 %). Hemotransfusion complications in 12 cases (44.4 %) were associated with of ABO-incompatible transfusion; in 7 (25.9 %) cases - with transfusion of media infected with infection transmitted by hemotransmissive route; in 4 (14.8 %) cases - with transfusion of Rh-factor-incompatible; and in 4 (14.8 %) cases - with transfusion of hemolysed erythrocyte-containing blood components. It was shown that the most common complications of transfusion occurred in surgical (37.1 %) and obstetric departments (29.6 %). In 22.2 % of the cases, complications were detected in therapeutic departments; in 11.1 % - in departments of anesthesiology, intensive care and intensive care. Complications developed more often with transfusion of erythrocyte suspension and were observed in 21 (77.7 %) patients. The occurrence of posttransfusion complications was timely detected in 17 (85 %) patients who received emergency medical care, which led to a successful outcome. In 3 cases, a fatal outcome was recorded, which accounted for 11.1 % of the total number of complications. The causes of hemotransfusion complications are more often non-compliance with the rules for determining the blood group and Rh-factor, as well as non-compliance with the rules for conducting blood compatibility tests of the donor and recipient provided for in the instructions on hemotransfusion; therefore, it was concluded that it is necessary to increase the knowledge and training of medical personnel on transfusion therapy.

A spatial analysis of intra-regional transport accessibility of settlements in the Arctic districts of the Sakha Republic (Yakutia) by GIS is an urgent task and can become a necessary support for the processes of decision-making and planning of sustainable development of these territories. The purpose of the study is to perform a spatial analysis of intra- regional transport accessibility of settlements in remote Arctic regions of Yakutia on the example of Anabarsky and Nizhnekolymsky districts. On the basis of the GIS network analysis and calculated coe cients of density of the transport network, the assessment of transport accessibility of the study areas according to winter and summer scenarios was carried out. The automobile and river networks were built on the basis of data from open cartographic sources: OpenStreetMaps and Google Earth Engine, the atlas of highways of the Sakha Republic (Yakutia). On the basis of satellite images of Landsat 5, 8 and Sentinel 2, non- o cial (informal) roads that play an important role in communications between settlements were decrypted and digitized. QGIS and ArcGIS programs were used to perform network analysis and build a map of time availability of the settlements using isochronous method. The created maps clearly show the availability of settlements by time interval. The conducted research made it possible to determine that remote sensing and geoinformation systems are key tools for a spatial analysis of transport infrastructure. Using this tools, it is possible to conduct network analysis of various levels of complexity, as well as build graphs of the transport network, greatly simplifying the counting procedure. The paper tested a method that allows to identify uno cial roads in the Arctic, and the presented methods of GIS analysis and modeling that can be used for further deeper network analytics and graph theory to improve the methodology for studying the transport accessibility of the Republic of Sakha (Yakutia).
