In 2017, the Sakha Republic’s Occupational Pathology Center marks its 25th anniversary. A great contribution to its organization was made by Ara Andreevna Bezrodnykh, Honored Worker of Science of Yakutia, Doctor of Medical Sciences, Professor of the Medical and Treatment Faculty of the Yakutsk State University. The article describes the stages of development of the occupational medicine service in the Sakha Republic (Yakutia), its main achievements in scientific, organizational, medical-diagnostic, preventive, and educational work. Today the Sakha Republic’s Occupational Pathology Center is the only medical institution in the republic that provides the coordination and arrangement of medical examinations and consultations to persons working in harmful and dangerous work conditions, also to persons engaged in the work of the high-risk units. The centre examines any association of the disease with the profession and assesses professional suitability. The article presents the statistical data on the chronic occupational diseases in the Sakha Republic (Yakutia) in the recent years, their dynamics and structure. As of the beginning of 2017, more than 3,000 patients with various chronic diseases were registered in the Sakha Republic’s Occupational Pathology Center. In the period from 2012 to 2016, each year the Center identified from 114 to 199 new cases. The incidence of chronic occupational pathology varies from 2.5 to 4.4 cases per 10,000 working population, which is 1.5-2.5 times higher than the average all-Russian indicators. Among newly diagnosed patients in 2016, 79 % were working in mining enterprises, 17.1 % - transport and communications workers, 1 % - construction, 1 % - health care, etc. According to the analysis of the structure of the first identified occupational diseases for 2016, the leading place is occupied by diseases of respiratory organs - 26,2 %, hearing loss - 26 %, vibration disease -24.04 %, diseases of musculoskeletal system and peripheral nervous system from the functional surge - 22.6 %. In the recent years, there has been noted the increased pathology of the respiratory and musculoskeletal system and peripheral nervous system. The article presents the results of the diagnostic search of the hereditary autosomal recessive disease Pendred's syndrome (sensorineural deafness combined with thyroid disorders) in patients with hearing disorders in Buryatia using instrumental (threshold tone audiometry) and laboratory methods (ELISA analysis of FT3, FT4 and TSH). Threshold tonal audiometry and analysis of thyroid hormone levels were performed in 164 patients with hearing impairment. The analysis showed that 7.9% (13 out of 164 people) of patients with severe hearing loss and deafness can be assumed to have thyroid disorders (12 people – hypothyroidism, 1 person – hyperthyroidism). Overall, 7.3% of deaf patients with hypothyroidism (12 of 164) were formally consistent with the clinical features characteristic of Pendred's syndrome

In this paper we present for the first time the results of the clinical-audiological and genealogical analysis of cases of hearing impairment in the Republic of Buryatia. As a result of the clinical-audiological analysis of the survey sample, the majority of individuals had bilateral deafness of the sensorineural type, the rest had bilateral hearing loss of varying severity. The segregation analysis carried out in Buryat and Russian families made it possible to assume the hereditary nature of cases of hearing impairment, segregating according to an autosomal recessive type of inheritance only in Russian families

We conducted a questionnaire and a selection of buccal epithelium of people, whose mean age is 21, in order to analyze their opinion on the potential risk of a deaf child’s birth and conduct genetic testing for the presence of the mutation c.-23+1G>A in the GJB2 gene. Analysis of data from the questionnaire with genotypes shows that there are no statistically significant differences in the respondents’ responses (p>0.05). As a result of the questionnaire, it is shown that most young hearing people think that deafness can be a hereditary disease. Most young people assume the possibility of birth of a deaf child from hearing parents, but very few respondents agree with this risk. Such a response can be explained by the protective internals of the psyche, when a person assumes the existence of the same risk of the birth of a deaf child in all people. In heterozygous carriers of the mutation c.-23+1G>A in the gene GJB2, there is a tendency to assume the hereditary nature of deafness more often than in people without this mutation.Such a response can be explained by the presence of close deaf/ hard-of-hearing relatives.

Autosomal recessive deafness type 1A is one of the most frequent hereditary diseases in the Republic Sakha (Yakutia). The revealed features of the spectrum and frequency of pathogenic variants in the GJB2 gene in patients with congenital hearing impairment allow introducing routine DNA diagnostics into medical practice. The article discusses some of the bioethical issues of DNA testing of autosomal recessive deafness 1A type.
